DOT Substance Abuse Professional (SAP) Evaluations

DOT drug or alcohol violation can temporarily remove you from performing safety-sensitive job duties, but it does not have to end your career. Completing the Return-to-Duty process with a qualified DOT Substance Abuse Professional (SAP) is an important step toward returning to work.

I provide DOT SAP evaluations in accordance with 49 CFR Part 40 for employees working in DOT-regulated safety-sensitive positions.

Who Needs an SAP Evaluation?

  • Aviation (FAA)

  • Railroads (FRA)

  • Public Transit (FTA)

  • Pipelines (PHMSA)

  • Maritime (USCG)

Evaluation Fees

Initial SAP Evaluation (includes follow-up evaluation): $500

Cancellation Policy

  • Cancellations with less than 24 hours' notice: $250

  • Missed appointments (No Show): $250

The Return-to-Duty Process

The SAP process follows federal DOT regulations and generally includes the following steps:

Initial SAP Evaluation

A comprehensive clinical evaluation conducted through a secure HIPAA-compliant telehealth platform to understand your history, current circumstances, and determine the appropriate education and/or treatment recommendations.

Education and/or Treatment

Based on the evaluation, individualized recommendations are provided in accordance with DOT regulations. These recommendations must be completed before moving to the next stage.

Follow-Up Evaluation

After completing the recommended education and/or treatment, a follow-up evaluation determines whether you have successfully complied with the SAP recommendations and are eligible to continue the Return-to-Duty process.

Return-to-Duty & Follow-Up Testing

When appropriate, I provide documentation to your Designated Employer Representative (DER) and establish the federally required follow-up testing plan in accordance with DOT regulations.
