Notice to Clients: Access to Records & Complaints

Your Right of Access to Your Records

Under Texas law, you have the right to request access to your mental health records held by this practice. This includes the right to examine and receive copies of records concerning your diagnosis, evaluation, and treatment.

To request access, you must submit your request in writing to our office. We will respond within 15 business days of receiving a written request and provide access or copies of your records unless a portion of the record is legally exempt from disclosure.

Reasonable fees may be charged for copying and mailing records, as permitted by law.

If we deny access to any portion of the record for clinical reasons, we will provide a signed and dated written statement explaining the denial and specifying the portion of the record withheld, the reason for withholding, and the expected duration of the denial.

Complaints

If you believe your rights regarding access to records or other aspects of your care have been violated, you may file a complaint with one or more of the following agencies:

Texas Behavioral Health Executive Council (BHEC)
Complaints against licensed behavioral health providers (such as social workers, counselors, therapists) can be filed with the Texas Behavioral Health Executive Council.

Complaints related to privacy rights or medical record privacy may be filed with the Texas Attorney General’s Consumer Protection Division.
